PhilHealth for OFWs: Are Your Parents and Kids Really Covered Back Home?

Many overseas Filipino workers assume that paying PhilHealth premiums automatically protects their parents and children in the Philippines. It does not. Coverage for family members depends on whether they are properly declared as dependents under the OFW’s PhilHealth record.

PhilHealth Advisory No. 2026-0042, signed on July 21, 2026, outlines the benefits available to OFWs and their dependents abroad, including reimbursement for overseas hospital confinement and the rules on premium contributions.

The advisory, which PhilHealth regional offices also posted on Facebook, states that OFWs and their dependents “wherever they are abroad” may claim reimbursement for overseas confinements under the applicable PhilHealth All Case Rate. Claims must be filed within 180 days from the date of discharge.

Who Counts As A Dependent

PhilHealth recognizes the following as qualified dependents of a principal member:

  • A legal spouse who is not enrolled as a PhilHealth member.
  • Legitimate, legitimated, acknowledged, or illegitimate children, as well as adopted or stepchildren, who are below 21, unmarried, and unemployed.
  • Children aged 21 or older with a congenital or acquired disability that leaves them totally dependent on the member, as determined by PhilHealth.
  • Foster children as defined under Republic Act No. 10165, or the Foster Care Act of 2012.
  • Parents aged 60 or older who are not PhilHealth members and whose monthly income falls below PhilHealth’s threshold.
  • Parents with a permanent disability that makes them totally dependent on the member, regardless of age.

Coverage Is Not Automatic

An OFW’s membership alone does not guarantee that every parent or child is covered. PhilHealth requires qualified dependents to be declared by the principal member, with their names appearing in the Member Data Record or MDR.

  • Check that each dependent’s name appears in the MDR.
  • Confirm that children still meet the age, marital status, and employment requirements.
  • Verify that parents meet the age, income, membership, or disability conditions.
  • Keep premium contributions updated, since contribution status can affect benefit access.

The 45-Day Limit Is Shared

Qualified dependents are entitled to up to 45 days of coverage per calendar year—but the allowance is shared among all qualified dependents, not granted separately to each person.

For example, an OFW with three qualified dependents still has a combined limit of 45 days for the year under this allowance.

What OFWs Can Claim Abroad

For hospital confinement overseas, PhilHealth provides reimbursement based on the applicable All Case Rate, rather than payment of the full foreign hospital bill. The reimbursement claim must be submitted within 180 days from discharge.

When OFWs are in the Philippines, they may also access PhilHealth benefits and services from contracted providers, including the Yaman ng Kalusugan Program (YAKAP) and Z Benefits. The advisory does not say that all medical expenses are fully covered, so members should still confirm eligibility and claim requirements through official PhilHealth channels.

Premium Payments

As direct contributors, OFWs must pay premiums equivalent to 5% of their monthly income, effective January 2025. The minimum monthly contribution is ₱500.

PhilHealth says premiums may be paid monthly, quarterly, semi-annually, or annually through accredited collecting agents in the Philippines and overseas. Members are advised to check whether payments have been posted through the PhilHealth Member Portal or the eGov PH Super App.

What OFWs Should Do Now

  • Review the PhilHealth MDR and confirm that a spouse, children, or qualifying parents are listed as dependents.
  • Check whether children still meet the below-21, unmarried, and unemployed requirements.
  • Confirm that parents meet the age, income, membership, or permanent-disability conditions.
  • Keep the 5% monthly contribution updated, with the ₱500 minimum in mind.
  • File any overseas-confinement reimbursement claim within 180 days of discharge.
  • Verify membership and dependent details only through PhilHealth’s official channels before assuming family members back home are covered.
